Subset Description :
Codes to record DNACPR status, and awareness of the clinical decision, for Advance Care Planning in accordance with MCA 2005. The senior responsible clinician for a person may record their opinion and recommendation for future management should a cardiopulmonary arrest occur as ‘Do not attempt cardiopulmonary resuscitation’ (DNACPR) orders in the following circumstances: 1. Where cardiac or respiratory arrest is an expected part of the dying process and it is the opinion of the senior responsible clinician that attempted cardiopulmonary resuscitation would be unsuccessful or the outcome would be unacceptably burdensome for the person. 2. Where a person has given instructions that, in the event of cardiopulmonary arrest, they do not want attempted cardiopulmonary resuscitation.

To ensure that all healthcare professionals involved in an individual’s care are aware of the decision (and patient's awareness of clinical decision) that an attempt at cardiopulmonary resuscitation would not be successful or would not be in the person’s best interests. Recording of DNACPR decisions will support professional decision making should a cardiopulmonary arrest occur and will avoid the carrying out of invasive procedures on a person when a clinical decision has already been made that to attempt resuscitation would not be in the person’s interest. The National End of Life Care Intelligence Network, hosted by Public Health England, has undertaken widespread consultation and updated to SCCI1580 following formal withdrawal of support for the Liverpool Care Pathway. The related message specification and value sets have been updated and a number of subsets have been created to support this requirement. Many of them will be useful for other purposes and thus where wider utility is expected the names do not reference “end of life care”.
